Sports Schools - Program Assessment Pre-Task

In conjunction with the CSSHL, Hockey Alberta is working with pre-determined Sports Schools each season to complete a quality assessment of the organization’s programming. The Program Assessment will consist of this pre-task survey and an in-person meeting to discuss and evaluate the current state of the program’s operations. Hockey Alberta will reach out to you to set up the in-person meeting/interview upon the completion of this pre-task survey. The information compiled throughout the process will provide feedback to your Sports School on your current areas of strength and areas for improvement.

Please take some time to complete this pre-task survey by selecting your response in each area and uploading the necessary supporting documentation. The form may be saved and completed at a later time; please ensure you keep a copy of the web link and password to access your form. Once your click SUBMIT, the form will be submitted directly to Hockey Alberta and we will initiate the next steps of the Program Assessment process.
