Minor Hockey Overage Player Application Form

Players who meet Hockey Alberta’s overage eligibility criteria should contact their Minor Hockey Association (MHA) regarding registration. This application is intended for MHAs seeking an exception to Hockey Alberta’s Overage Player Policy on behalf of a player who does not meet the overage eligibility criteria.

Applications must be submitted by the MHA and are accepted between August 15 and October 31 of the current season.

NOTES PERTAINING TO THIS FORM:

• Required Fields: All fields noted with an asterisk (*) must be completed.
• Upload Files: When uploading files, preferred file format is PDF. Only single files can be uploaded. If more than one document is required, it may be necessary to merge into one file.
