Board of Directors - Nomination Form
The Alberta Amateur Hockey Association (Hockey Alberta) is seeking qualified candidates for three (3) positions of Director at Large on its Board of Directors. The Board is accountable to its Members, ensuring that the mission, vision and values of Hockey Alberta are carried out by the organization. As a Director at Large, you are expected to commit to a three-year term on the Board. The Board meets regularly (in-person and online) and Board members attend events throughout the year. If you are interested and would like to help shape the future of hockey in Alberta, please complete all sections of the online Nomination Form and submit via the Hockey Alberta website. You may include a copy of your resume highlighting your career and volunteer history, and a cover letter outlining what you believe you can contribute to the Board of Directors of Hockey Alberta.

Deadline for nominations to be received by Hockey Alberta is November 11, 2021 at 11:59 PM (MT).
The Nominee is to fill out this form completely and submit by the deadline. Please email [email protected] immediately upon submission to confirm receipt of nomination.
The Nominee should confirm his/her email address appears accurately in the Forward Email section at the bottom of the form. This ensures the Nominee receives a copy of the submission.
Per Hockey Alberta’s Bylaws, a candidate for the Board of Directors must be nominated by a Hockey Alberta Member. The Nominee is to enter the email address for the Hockey Alberta Member representative who is making the nomination. The Nominator will receive an email, with a weblink and password. S/he is to go to that link, and complete the required information to confirm and finalize the nomination.
