The Alberta Amateur Hockey Association (Hockey Alberta) is seeking qualified candidates to the positions of Chair and Vice Chair on its Board of Directors. The Board is accountable to its Members, ensuring that the mission and values of Hockey Alberta are carried out by the organization. As a director, including the Chair and Vice Chair, you are expected to commit to a three-year term on the Board. The Board meets in person six times annually in Red Deer, holds a monthly conference call, and Board members attend events throughout the year. If you are interested and would like to help shape the future of hockey in Alberta, please complete this online Nomination Form (including Credentials Summary and Conflict of Interest declaration) and submit via the Hockey Alberta website. You may also upload a copy of your resume highlighting your career and volunteer history, and a cover letter recognizing what you believe you can contribute to the Board of Directors of Hockey Alberta.

  1. Deadline for nominations to be received by Hockey Alberta is Wednesday, July 31 at 11:59 PM (MT).

  2. The Nominee is to fill out this form completely and submit by the deadline.

  3. In the NOMINATOR INFORMATION section, the Nominee is to enter the email address for the Member representative who is making the nomination. The Nominator will be receive an email, with a weblink and password. S/he is to go to that link, and complete the required information to confirm and finalize the nomination. This process is done in lieu of a signature on the online form. Until the Nominator submits his/her confirmation, the Nomination is not complete.

  4. You may save the form at any time by clicking the SAVE FOR LATER button at the bottom of the form. Please ensure you keep a record of the unique weblink and password for your form so that you can access it again.

  5. To keep a copy of your form, enter your email address in the FORWARD SUBMISSION field at the bottom of the form.
