AFHL Franchise - Program Assessment Pre-Task
In conjunction with the AFHL, Hockey Alberta is working with AFHL franchises at the end of each hosting agreement term to complete a quality assessment of the organization’s programming. The Program Assessment consists of this pre-task survey and an in-person meeting to discuss and evaluate the current state of the program’s operations. Hockey Alberta will reach out to you to set up the in-person meeting/interview upon completion of this pre-task survey. The information compiled throughout the process will provide feedback to your AFHL franchise on your current areas of strength and areas for improvement.
Please take some time to complete this pre-task survey by selecting your response in each area and uploading the necessary supporting documentation. The form can be completed in more than one sitting by clicking SAVE FOR LATER. If you are unable to complete in one sitting, please ensure you copy and save the survey link and password to continue the survey at a later time. Once you click SUBMIT, the form will be transmitted directly to Hockey Alberta and we will initiate the next steps of the Program Assessment process.

If your association is looking to expand upon the current number of teams hosted, you will need to fill out this renewal form for your existing programs as well as the New Franchise Application.
